Medical Records & Form of Authority
The term "medical records" can cover a wide range of documents that may capture any treatment or medical consultations that you may have had at some point. The primary set of records are those held by your General Practitioner and any Hospital visits to treat your injuries following the material accident or injury. However, it may be necessary to obtain records from your Physiotherapists, Occupationaal Therapy records, X-rays etc.
In order for us to obtain your medical records, we will need you to sign a form of authority (sometimes called a "consent form"), releasing your records to us.
